Introduction: Laparoscopic surgery has been established as a leading minimally invasive approach in the treatment of colorectal cancer, demonstrating comparable oncological efficacy and significantly improved perioperative outcomes compared to open surgery. Aim: To present contemporary aspects, oncological principles, technological advancements, and future trends in laparoscopic colorectal surgery based on available scientific evidence and international guidelines. Materials and Methods: A review was performed of key randomized trials, meta-analyses, clinical guidelines, and recent publications evaluating the efficacy, safety, and development of laparoscopic surgery in colorectal cancer treatment. Results: Current evidence demonstrates that laparoscopic surgery provides equivalent oncological radicality, adequate lymphadenectomy, and R0 resections, while reducing surgical trauma, postoperative pain, hospital stay, and complications. Integration of ERAS protocols and robotic platforms further enhances postoperative recovery and functional outcomes. Data indicate advantages of robotic surgery in low anterior resections and anatomically complex cases. Conclusion:
Laparoscopic colorectal surgery is a standardized and proven effective treatment modality, based on oncological principles and technological progress. The combination of minimally invasive techniques, ERAS protocols, and robotic assistance defines the modern approach to optimal perioperative care and high oncological safety. Expansion of training programs, centralization of care, and adoption of innovations such as intraoperative fluorescence imaging and artificial intelligence are key for further development of the field.
