Pemphigus vulgaris (PV) is a chronic IgG-mediated autoimmune blistering dermatosis characterized by acantholysis resulting from autoantibodies directed against desmosomal adhesion molecules. Despite the establishment of rituximab as the first-line standard of care, a clinically significant subset of patients develops persistent, treatment-refractory erosions. We present the clinical experience of the Immunobullous Diseases Unit at the Department of Dermatology and Venereology, Aleksandrovska University Hospital, with the use of a topical therapeutic regimen incorporating Entan Derma Spray (grape seed proanthocyanidins) and Gingira Medicasol (10% Centella asiatica triterpenes) in PV patients presenting with refractory to conventional systemic and topical therapies skin lesions of the face and scalp. A therapeutic protocol consisting of a five-step topical treatment with Entan Derma Spray and Gingira Medicasol, combined with antimicrobial and keratoplastic agents, is described.
