Over the past 20 years, significant improvements have been made in the therapy of hepatocellular carcinoma, which in turn poses challenges in determining the correct treatment approach. Since the 1990s, the BCLC system has offered a structured approach based on disease stage to select the most appropriate therapy. In the age of personalized medicine and ever-evolving therapeutic options, the question arises as to whether this system is the most optimal for the patient. In this regard, a new concept – a multiparametric therapeutic hierarchy – has been developed. In essence, this proposal offers the patient the therapies known to provide the greatest survival benefit based on individual treatment choices. At the core of both systems is the optimal work of a qualified multidisciplinary tumor board.
