In the group of inflammatory demyelinating diseases of the central nervous system (CNS), different and independent pathophysiological mechanisms have been described, leading to different patterns of spinal cord damage. Involvement of the spinal cord is often associated with disability in patients suffering from multiple sclerosis (MS) or neuromyelitis optica spectrum disorders (NMOSD). In this review, we focus on the differences between the main groups of these demyelinating CNS diseases. Emphasis is placed on the supporting role of immunological and imaging studies on distinguishing between the different conditions. The need to establish an interdisciplinary approach in the diagnosis and follow-up of patients suffering from inflammatory demyelinating myelopathy is underlined.
