Cochlear implantation in patients with chronic otitis media or previous middle ear surgery poses several problems for the otosurgeon: possible spread of infection into the cochlea and subarachnoid spaces with subsequent meningitis, risk of extrusion of the active electrode of the cochlear implant, and possible relapse of the original disease.
Although in the past cochlear implantation in the condition of chronic otitis media with or without cholesteatoma was considered a contraindication, this has now been completely refuted.
