Modern cataract surgery goes beyond a standard medical intervention and has evolved into a refractive procedure aimed at achieving visual independence from glasses. Patients today seek not only restored vision but also functional and comfortable eyesight tailored to their lifestyle. A critical element of success is the selection of the appropriate intraocular lens (IOL), with premium options such as multifocal and extended depth of focus (EDOF) lenses receiving particular attention. This article analyzes their optical principles, advantages, drawbacks, and ideal patient profiles. Multifocal IOLs provide vision at multiple distances but are often associated with visual disturbances. EDOF lenses offer better visual comfort and higher tolerance-especially for night driving-at the cost of moderate near vision. The importance of an individualized approach, clear communication, and the role of general practitioners in the process is emphasized. In conclusion, successful cataract surgery requires not only precise technique but also proper expectation management and personalized lens selection.
