Lightning related electric shock may result to mortality (20-30%), and to high morbidity, that can lead to permanent sequelae. The risk of developing ocular findings is high as 50% after a lightning strike. Ophthalmological examination is particularly important for screening any potential injury to the eyes. We report a rare case of ocular injury after lightning accident on 10 August 1995, that was followed for 30 years: 1st two years according to available documentation, and last 8 years – by us. She had asymmetric anterior and posterior segments damage in both eyes. It is important to differentiate between stroke related pathological changes or by other causes in order to apply specific treatment approach. Patients should be regularly followed in eye clinics in terms of early, mid and late pathologies that may develop after lightning strike.
