The significance of skin microbiota dysfunction in the prognosis and management of seborrheic dermatitis

The inflammatory dermatoses most often occur in the context of cutaneous immune mechanisms imbalance, causing impaired keratinocyte differentiation, changes in sebum composition and invasion of pathogenic microorganisms. This complex pathogenesis predisposes to a chronic-recurrent course of the inflammatory process with formation of characteristic erythemo-infiltrative plaques and intense pruritus. The current scientific concept accepts genetic predisposition, along with environmental epigenetic factors and immune response imbalance as interrelated in the initiation and chronification of dermatological inflammation. The main pathogenetic immune-mediated mechanisms include impaired protein synthesis, oxidative cellular stress, increased expression of the nuclear factor NF-κB, and imbalance of adaptive immunity. Dysfunction of the skin microbiota in combination with T-cell mediated cellular signals are considered as main prerequisite for the occurrence of the dermatological inflammatory process. We, herein,present the most recent understanding of skin microbiota dysfunction involved in the complex pathogenesis of seborrheic dermatitis.
