The metabolic syndrome is a complex pathology of interconnected abnormalities of different organs and systems, which leads finally to disruption of the internal homeostasis and chronic inflammatory milieu. The skin has always been reflection of the internal status and the connection between internal diseases, the metabolic syndrome and the skin is bidirectional. Fоr the last several decades data has been collected that there is strong association between the risk factors, genetic predisposition, dysregulation of adipokine secretion, chronic low-grade inflammation and common pathogenetic pathways of the metabolic syndrome and psoriasis. Patients with these pathologies share common clinical features as abdominal obesity, hypertension, dyslipidaemia and insulin resistance. Eventually these both conditions lead to atherosclerosis, coronary artery disease and very high incidence of cardio-vascular incidents. Recent data reports increased risk of non-alcoholic fatty liver disease and renal impairment in patients with these comorbidities. The contemporary biological therapies provide opportunities to modulate the therapy of the metabolic syndrome and personalize the therapeutic approach.
