Frontal fibrosing alopecia – a diagnostic and therapeutic challenge

Frontal fibrosing alopecia (FFA) is a chronic, progressive, primary lymphocytic scarring alopecia characterized by recession of the frontotemporal hairline and often accompanied by eyebrow loss. The condition predominantly affects postmenopausal women, though it is increasingly being diagnosed in younger female patients and in men. The pathogenesis of FFA remains incompletely understood, with autoimmune mechanisms, dysfunction of follicular stem cells, hormonal imbalances, and various external factors including cosmetics, silicone implants, and others believed to play a key role. Diagnosis is based on a combination of clinical presentation, dermoscopy, and histopathology. Treatment involves a combination of anti-inflammatory, immunosuppressive, and antiandrogenic agents. The aim of present review is to summarize the current knowledge regarding etiology, pathogenesis, associated conditions, and therapeutic approaches in FFA.
