Leukocytoclastic vasculitis is the most common manifestation of inflammatory diseases of the vascular wall in clinical dermatological practice. A rapid progression of the pathological exanthema of the skin is often observed within a few days, which necessitates hospital treatment. The etiological factors leading to this disease are diverse, including viral, bacterial, fungal agents, as well as a large number of medications. It is relatively rare in practice to observe the development of leukocytoclastic vasculitis of the skin in a patient with late latent syphilis, as is the case presented. Thanks to this finding, confirmed by the performed laboratory tests and the combined corticosteroid and antibiotic treatment, the patient showed a good and relatively rapid improvement in the dermatological status. The observed therapeutic outcome correlated with the decrease in the titer of specific antibodies against T. pallidum. I believe that the presented case shows not only the many faces of syphilis as a „Great Imitator“ in medicine, but also the diagnostic challenges that leukocytoclastic vasculitis imposes.
