Abstract
Diabetic nephropathy (DN) is the main cause of chronic kidney disease. Despite numerous studies, its pathogenesis is still not fully understood. Microalbuminuria is the main established biomarker for the detection and prediction of DN and cardiovascular risk in type 1 and type 2 diabetes mellitus, despite the number of limitations that this method has. For this reason, in recent years, extensive studies have been conducted on novel biomarkers that have higher sensitivity and specificity for earlier detection of DN, as well as for more precise prediction of progression to CKD. Such biomarkers are: urinary nephrin, urinary transferrin, urinary tumor necrosis factor-alpha (TNF-alpha), urinary collagen type IV. Their high perspective in the diagnosis of DN is confirmed by the study conducted at the University Pediatric Hospital in Sofia “Prof. Ivan Mitev” among 85 adolescents with type 1 diabetes.
