Perinephric abcess represents collection of pus in repireanl space, confined between renal fascia and Gerota fascia. It is a rare, but potentially lethal condition. The reported mortality rate is between 39% and 50% irrespective of aggressive drainage of the cavity of the pus collection. The most common etiology is complicated urinary tract infection (above 75% of the cases), leading to fat tissue necrosis. The inflammatory process is extensive and could go beyond Gerota fascia, advancing towards m. psoas and mm. transversalis, peritoneal cavity and pelvis. Non-specific clinical presentation and lack of pathognomonic signs at physical examination makes the timely and exact diagnosis a challenge. Early diagnosis is a key step in reducing mortality and improvement of therapeutic results.
