Prostate gland biopsy – 20 years of experience – why, when and what?

Biopsy is a proof-of-concept method for carcinoma of the prostate gland. Modern diagnostics strives for early detection of cancer, but also protection from overdiagnosis. Biopsy candidates are patients with an expected survival of more than 10 years, as well as those with advanced carcinoma at the time of diagnosis. The main methods for determining indications for biopsy are the total prostate-specific antigen and a positive finding from rectal palpation of the prostate gland. Prostate cancer is a multifocal process. The prostate gland has different anatomical zones and areas in which carcinoma of different histological grades can be found. The areas are in anatomical relation with various neighboring structures and tissues. Due to these circumstances, the biopsy technique must take tissue material sufficient in volume and quality, distributed and described according to the areas from which it was taken. With the received information, a treatment method is selected and its result is predicted. The method is primarily performed through transrectal access. In the last few years, perineal access has also entered. In our practice, from 2003 until now, only transrectal puncture biopsy of the prostate gland has been performed.
