Macrocephaly is a common condition defined as a head circumference greater than the 97th percentile or 2 standard deviations above the mean for their age (above +2SD). The differential diagnosis of macrocephaly is broad, ranging from benign familial macrocephaly to rare metabolic disorders. A large number of disorders can affect the brain parenchyma, intra- and extraventricular cerebrospinal fluid spaces, and the intracranial blood pool, making the clinical diagnosis of macrocephaly nonspecific. Macrocephaly must be distinguished from megalencephaly, which is due to true enlargement of the brain parenchyma. This article presents the main disorders that present with macrocephaly, categorizing the megalencephaly group.
